It will be OK--two weeks is tons of time to get everything done. Of course, I was in a major panic about Lyra's birthday a month and a half ago, so I know of what you speak. :-) Breathe deeply. It will be OK.
Here's my list to help get you started. I baked and decorated my own cake, but the party didn't have a theme other than purple.
FYI, two dozen bagels is waaaaay too much for five toddlers and their parents. We also ended up getting hummus, baba ganoush, etc., and mini pitas at Trader Joe's, so we had waaaaay too much food. I also forgot to put out the fruit.
We had a hot, beautifully sunny day, so we ended up having a pool party in the wading pool.
Things to do for Lyra's birthday:

Party favors and bags to put them in
Decorations (balloons?)
Paper plates and bowls (check on current supply)
Plastic forks and spoons

Touch up cleaning (vacuuming if needed downstairs)
Get bagels (Go to Sammy's)
